Step
Three: Print
a blank form to work on.
Step
Four: Read through the form
several times to familiarize yourself with its content and
organization. Pay careful attention to the instructions
throughout the form.
Step
Five: Use
the blank form to begin filling out a handwritten "rough"
version. You can use this form to make notes, corrections,
and so on. You might want to start by filling in the "easy"
information ... the fill-in the blank front page, check boxes,
list of DeMolay offices, and so on, and then going back to
write the more detailed sections. Or, you may find it easiest
to work through the form one section at a time. Don't try
to finish it in one sitting ... this is something you need
to spend some time on and think through
carefully.
Step
Six: Print
an extra copy of the "Parental Evaluation"
section (p. 12 and 13) and give it to your parents. Explain
that you or they will need to enter this information on the
final copy of your form later, but that they can use this
copy for a "rough draft" if they like.
Step
Seven: At some point during the process of filling
out the form, you'll need to get together with an Advisor
in your Chapter to complete the "Self-Mastery"
evaluation on page 17. Again, you may wish to print
an extra copy of the page to give to him in advance. You should
also explain to your Advisor that you'll be transferring the
responses onto the final form later, and coming back to him
for his signature then.
Step
Eight: Be sure to
complete the grey-boxed self-evaluation in each section
of the form.
Step
Eight: Talk
with your RD Advisor or Chapter Advisor about your
progress on the form. When you complete your "rough draft"
answers, meet with him to review your work before
typing out the final form... you don't want to submit a form
to your Advisory Council that they won't approve! This is
also the time to check your spelling and grammar.
Step
Nine: When
you are entirely satisfied with your responses to all the
areas of the form, open a copy of the on-line
"fill-in" form (or you may print another
blank form to use in a typewriter ... neatly handwritten forms
will be accepted, but are not preferred). If you will be entering
the information your parents entered in their evaluation,
be sure to have it handy. Or, you may leave that section blank
and have them fill it in before submitting the form.
REMEMBER
that you won't be able to save your work on
the fill-in form. When you close this copy of the form, you
will lose all the information you've entered, so don't
begin typing on this form until you are ready to complete
it! Alternately, you might open the form, fill-in
and print section one, close the form, do section two later
on, etc...
Step
Ten: After
you've filled out and printed the complete form, go back to
the Advisor you completed the "Self-Mastery"
section with and obtain his signature. Be sure to
get your parents to sign their evaluation section
as well. The form requires your signature on
the "Self-Mastery" section, as well as on the front
page.
Step
Eleven: Attach
a copy of your report card to the completed
form. You should also attach any additional materials
you'd like to have considered. Remember to attach copies,
not originals --your application will not be returned
to you. If you use additional pages to answer any questions
on the form, attach these, marking responses clearly
with the page number and question number they are
answering.
Step
Twelve: When
all these materials are together, you're ready to present
your application to your Advisory Council for approval.
Make a copy of all the materials before you let it
out of your sight! It would be a shame to have to
start over if someone misplaced your work. Give the original
form and supporting materials to your Chapter Advisor,
who will obtain the necessary Advisory Council approval. Remind
him that two Advisors signatures are needed on page
one, and that page 30 contains additional space
for any comments he (or the RD Advisor) wish to add.
Please do not leave this page blank... if he has no additional
comments, mark the box "None." If the Advisor does
add comments on page 30, he must sign that page at the bottom
as well.
Step
Thirteen:
When you get the form back from your Chapter Advisor, make
a copy of the Advisor's comments page and the front page with
Advisors signatures, and put them with your file copy
of the form.
Step
Fourteen:
Make one last check to see that nothing is missing
from your application. Go over the checklist on the back page
of the form.
Step
Fifteen: Put all of the original
materials in a large envelope ... do not fold them
... and mail them to Pennsylvania DeMolay; 1244 Bainbridge
Road; Elizabethtown, PA 17022. It might be wise to
send the package Certified Mail, or Return Receipt Requested,
to ensure that we've gotten it.
