Several other jurisdictions have asked us about an RD form they can use, similar to the new form used in PA. As a service to our brother jurisdictions, we're now offering a general form for use outside of PA. Any jurisdiction is welcome to use this form without special permission, provided it is not changed in any way. PA DeMolay retains ownership of this form.

The "new" PA-style RD form has been redesigned with younger members in mind, and asks questions more relevant to all-around development in the young man of today. It asks members to evaluate themselves in 6 areas:

• Mental development
• Physical development
• Social development
• Emotional development
• Spiritual development
• DeMolay activity

To use this form, please take note of the following items:

1. Do NOT use this form if you are a PA DeMolay. Click here for instructions for PA DeMolays, including links to the proper form.

2. In order to use this form in your jurisdiction, your Executive Officer must approve its use. If you are unsure whether this form is in use in your jurisdiction, contact your Executive Officer first.

3. There are two .pdf files available to you. Both will require that you have Adobe Acrobat Reader installed on your computer. Version 5.0 or later is recommended.

4. The "Blank" form is for your use to print out and work on as a "rough draft" or for use if you will handwrite or type your form on a typewriter.

5. The "Fill-in" form will allow you to type your responses directly onto the form using your computer, and then print out the completed form. Please note that, unless you are running the full (purchased) version of Adobe Acrobat, you will be unable to save your work. We recommend you type and print one page at a time so as to avoid losing information and having to retype it.

6. Please note that special instructions or additional attachments may be required by your jurisdiction. Contact your jurisdictional Executive Officer or his representative for details.

7. DO NOT MAIL THIS FORM TO PA DeMOLAY. Contact your Executive Officer or his representative for the proper place to mail your completed form.
