Elected State Officer Chapter Event Report

This report is to be filed with the State Master Councilor within 48 hours of each visitation, phone, or in-person contact with a PA Chapter. You should also forward copies of any e-mail or standard mail contacts to the State Master Councilor, if they are relevant to state business or will help to evaluate the Chapter's health.
